Follow the instructions in Section B and complete the section that relates to your particular circumstances: Medical or Non-medical.

It is important that Section B is completed before you bring Section A back to the school. This process should be started as soon as you are aware that you will require a derived grade.

THIS FORM MUST BE DOWNLOADED AND TAKEN TO THE MEDICAL PRACTITIONER IF THIS IS AN APPLICATION DUE TO MEDICAL REASONS.

Pass the completed form on to the Principal's Nominee (Mr Govind) before Friday 2 December.
